Wellness center offers an array of physical examinations such as annual and pre-employment/routine physicals along with gynecology check-ups, well baby program and mandatory civilian physical examinations to keep local and foreigners healthy and fit.

Various immunization shots are also offered for VISA applicants and students wishing to study abroad.

Please answer all questions listed on the questionnaire.

1) Finish evening meal before 8:00p.m. and no consumption of food thereafter.
2) Do not eat, drink or smoke on the morning of examination.
3) Arrive at receptionist desk between 7:30-8:00a.m. for registration.
4) Bring all questionnaire, urine and stool samples along with insurance card(if any).
